I just happen to have some fiber quotes from the telco's sitting on my
desk.  They're in the $2000-$2400/month range for unlimited GB over 5
year contract.  Installation is around $2000-$3000 if they don't have
to dig any trenches (or sometimes they'll absorb the trenching cost
but its about $15000 per city block).  IP addresses are free, but you
have to justify their usage.

I don't know of any other commercial grade options.
